Let's assume that you have four possible answers to your question, for the user to choose just one of them. You will represent them as a group of checkboxes with the answer as its text and layout them in two columns.
So you start by selecting the "question_box" container and then adding a new container in it (just after the image) to become the container of the two columns of answers. Let's rename the new container to "answers_box" and select it to set its attributes as follows:
Then, you need to add two new containers to the "answers_box" for the two columns. Let's rename those two to "left_column" and "right_column" respectively. Our tree should look like this:
Now select the "left_column" to set the following attributes:
You can copy the attributes of the "left_column" to the "right_column" by first selecting the "left_column" and then clicking on the "copy attributes" button. Then you should click on the "right_column" and click on the "paste attributes" button:
Now let's add two checkboxes ("Material Checkbox" in the component menu) to each of the "column" containers then rename them to "answer_1", "answer_2", "answer_3" and "answer_4 respectively (1,2 in the left column and 3,4 in the right column).
Select "answer_1" and set the following attributes:

Then copy its attributes and paste them (as shown earlier) to the remaining checkboxes. Finally set their label to "2. Mouse", "3. Cat" and "4. Dog" respectively. Finally set their attributes "checkbox-background" and "checkbox-checked-background" as shown for "answer_1" above. This is how it should look like in the end:
